We are looking to engage highly skilled Complex Case Managers / Clinical Case Managers to deliver trauma-informed case management services. In this role, you will manage a caseload of clients with complex needs, providing high-quality support and facilitating access to health, mental health, and psychosocial services. This is an initial 3-month contract, with potential extensions, based across multiple locations Australia-wide. Flexible working arrangements are also available.

The successful Complex Case Managers / Clinical Case Managers will be responsible for, but not limited to:

• Undertake intake and risk assessments and develop tailored wellbeing plans.
• Provide specialised case management and clinical advice to enhance service delivery.
• Build and maintain strong relationships with internal teams and external stakeholders, including community organisations and service providers.
• Escalate high-risk issues and contribute to high-level client summaries and briefs.
• Act as the primary point of contact for allocated clients, coordinating access to services and supports.

The successful Complex Case Managers / Clinical Case Managers must have:

• Tertiary qualifications in Nursing, Psychology, Occupational Therapy, or Social Work.
• Professional experience working with individuals with complex support needs.
• Strong understanding of disability impacts and capacity-building models.
• Excellent communication skills and proficiency in IT systems.
• Experience in mental health services and familiarity with evidence-based therapeutic interventions will be highly regarded.
