A leading Federal Government agency is seeking an Clinical Care Coordinator to join a high-performing team. This critical role will provide short-term, goal-oriented clinical case management to clients with complex health and psychosocial needs, ensuring they can access and engage in mental health care.

The successful Clinical Care Coordinator will be offered an initial 24-month contract with a 12-month extension option at a competitive hourly rate.

The successful Clinical Care Coordinator will be responsible for, but not limited to, the following:

  • Deliver high-quality, evidence-informed clinical services including risk management, brief interventions, care coordination, and referrals.
  • Maintain accurate client records and care plans using the Client Management System.
  • Build strong relationships with internal teams and external stakeholders to ensure coordinated care.
  • Apply trauma-informed, client-centered approaches to intake, assessment, and intervention.
  • Participate in professional development, clinical supervision, and contribute to program evaluation and service improvement initiatives.

The successful Clinical Care Coordinator must have experience in the following:

  • Excellent interpersonal skills and competency in trauma-informed care.
  • Ability to complete clinically appropriate intake assessments and risk management plans.
  • High-level written and verbal communication skills.
  • Sound judgment and ability to respond to critical incidents calmly and effectively.
  • Strong IT skills and ability to navigate clinical systems.

Mandatory Requirements:

  • Ability to obtain Working with Vulnerable People or Working with Children check.
  • Relevant qualifications and registration:
    • Psychologist, Occupational Therapist, or Registered Nurse (AHPRA), OR 
    • Social Worker (AASW membership), OR
    • Masters-level counselling qualification with PACFA or ACA registration.
